Key differences

  • Administration: Gonadorelin — Subcutaneous injection, Intranasal (pulsatile delivery); MK-677 (Ibutamoren) — Oral (capsule/liquid).
  • Dosing units differ: Gonadorelin is dosed in mcg, MK-677 (Ibutamoren) in mg — they operate at different scales.
  • Frequency: Gonadorelin is typically 2–3x per week subcutaneous; MK-677 (Ibutamoren) is once daily oral.

How each works

Gonadorelin

Gonadorelin directly binds GnRH receptors in the anterior pituitary to trigger LH and FSH secretion, which in turn stimulates Leydig cells for testosterone production and maintains spermatogenesis. Used as a TRT adjunct, pulsatile gonadorelin administration mimics natural hypothalamic GnRH to prevent testicular suppression associated with exogenous androgen use.

MK-677 (Ibutamoren)

MK-677 binds the ghrelin receptor (GHS-R1a) to stimulate pulsatile GH release and increase circulating IGF-1. Unlike peptide GHRPs it survives oral administration. Clinical trials demonstrate significant increases in GH pulsatility and IGF-1 without suppression of the endogenous GH axis, though long-term use raises glucose metabolism concerns.
